A Culture Focused on Patients

Putting patients first is the cornerstone of our culture at Memorial Healthcare System—and it’s vital to the culture of all great hospitals. Keeping patient engagement top of mind, we coordinate care across the continuum, working together to ensure the best possible outcome.

patient- & family-centered care (PFCC)

At Memorial, we treat our patients and their families as true partners in healthcare. By sharing information, along with the tools and support they need, we help patients and their loved ones make meaningful choices about their care.
What does it mean to provide PFCC?
Understanding and respecting patients' cultural traditions, their personal preferences and values, their family situations and their lifestyles
Giving patients and their families the opportunity to make choices about many aspects of their care and the healthcare service delivery process
Involving patients and their loved ones as part of the care team, collaborating with healthcare professionals in making clinical decisions about their healthcare services
Putting responsibility for important aspects of self care and monitoring in patients' hands along with the tools and support they need
Ensuring that transitions between providers, departments and healthcare settings are respectful, coordinated and efficient
Gathering and respecting input from Patient and Family Advisory Councils

An integrated system

The sharing of patient data gives our providers access to information that improves patient care and safety at the point of care. Memorial Healthcare System utilizes the Electronic Health Record (EHR) system from Epic Systems Corporation. As an integrated EHR, patient data is accessible to our providers in the health system across ambulatory and acute settings.

Commitment to Healthy
Communities & Quality

As part of our philosophy to make our communities healthier, Memorial Healthcare System uses social determinants of health and other community data to identify patient and family needs, while supporting the expansion of private practices to meet those needs. Memorial’s leadership and innovation in quality and performance improvement was recently recognized by The American Hospital Association Quest for Quality Prize Opens a new window.
